Katy Independent School District (ISD) in Texas is notifying nearly 12,000 current and former employees that an Internal Revenue Service (IRS) agent misplaced a portable flash drive containing their personal information while conducting a random audit under the direction of the IRS.

What was the response? Katy ISD is following the IRS investigation of the incident and will seek recovery costs from the IRS for credit monitoring services it has retained for the affected employees. All potentially impacted individuals are being notified, and have been automatically enrolled in the credit monitoring program.
Details: The portable flash drive was misplaced on Aug. 5. The incident impacts current and former employees who received a paycheck, or made a contribution to a voluntary 457(b) or 403(b) plan during the 2013 calendar year only. According to reports, the portable flash drive has not been recovered.
Quote: “Most Katy ISD employees received notification of this incident from the District via email on August 6, while all employees will receive a letter from the District, with additional information about the credit monitoring services it retained, by way of postal mail this week,” a Katy ISD statement said.
